Case Management Services are community outreach services that assist seriously mentally ill (SMI) adult clients, focusing on improved self-sufficiency. Case Managers have knowledge of community services and resources that are relevant to the lives of the persons served.
Case Managers work in collaboration with the client to achieve goals toward independence including crisis management, community linkage, social supports, Activity of Daily Living (ADL) training, housing, employment, budgeting and health assistance.
Case Management Services are based at each of our main offices in Valparaiso, Portage and Knox and are tailored to the individual served.
