PROFESSIONAL/CLINICAL:
  

  

             

OUTPATIENT THERAPIST (LCSW)

  • Conducts individual, group and family therapy.

  • Completes all documentation in the electronic medical record as required by the agency's policies and procedures.

  • Monitors and follows up on treatment progress of clients by telephone or in writing (i.e. calls to schools, probation, parents, etc.).

  • Participates in staffings with psychiatrist to review assessment, diagnosis and treatment plan.

  • Attends peer consultations to discuss cases and treatment approaches.

  • Provides education through presentations for local schools or community organizations.

  • Meets established productivity goals.

  • A Master's Degree in Social Work is required.

  • Licensed Clinical Social Worker (LCSW) is required.

  • Two or more years' experience providing counseling services in a clinical setting or community mental health center is preferred.

 

ADULT DAY TREATMENT SPECIALIST – Location: Knox

  • Plans, organizes and provides a variety of group therapy experiences congruent with treatment plans.

  • Provides appropriate therapeutic interventions.

  • Initiates, monitors and updates clients' treatment plans per department standards and in cooperation with treatment team.

  • Assists clients in meeting their treatment goals.

  • Completes electronic medical record documentation as required.

  • Provides documentation of interventions per department standards.

  • Helps to maintain a safe and cleanly environment for staff/clients.

  • Drives/transports members in personal or agency vehicles as needed.

  • Creates group materials as needed.

  • Coordinates care with internal and external providers.

  • Attends staffings, clinical supervision, in-services and staff meetings.

  • Bachelor degree in psychology, social work or related human services field required.

  • Valid driver's license and ability to obtain public passenger chauffeur license.

 

DIRECTOR, STARKE COUNTY OFFICE – Location: Knox

  • Develops, maintains and modifies the array of treatment programs and community outreach services available to clients including Day Treatment, Case Management, Outpatient, Medical Services, Chemical Dependency and Addictions and Admissions/Emergency Services.

  • Evaluates the need for development of alternate programs in response to client and community needs.

  • Evaluates client responsiveness to treatment, and recommends changes in treatment to foster client's treatment goals.

  • Maintains necessary documentation of services in compliance with agency policy and procedures, regulatory bodies, state law, contract specifications and professional standards.

  • Coordinates thorough assessment of client's needs, limitations, clinical symptoms, and recommends and refers to appropriate programs (i.e. residential services, vocational rehabilitation services, educational opportunities, employment services, substance abuse programs, support groups, and other community services).

  • Performs clinical supervision functions in the absence of the Clinical Supervisor.

  • Writes, reviews, and modifies all policies and procedures related to essential documentation of services.

  • Serves as a liaison between the agency and other state hospitals, community mental health centers, or residential services to provide placement.

  • Provides linkage to emergency resources (i.e. temporary shelter, financial assistance, medication or hospitalization).

  • Provides client advocacy, particularly with respect to personal safety and access to appropriate support systems.

  • Collaborates with internal and external health care providers to maximize and promote continuity of care.

  • Represents the agency at community meetings and events to expand referral sources, monitor satisfaction and develop collaborations.

  • Acquires and maintains adequate levels of qualified staff to match the needs of programs and services.

DIRECTOR, COMMUNITY SUPPORT SERVICES – Location: Valparaiso

  • Develops, maintains and modifies the array of treatment programs and community outreach services available to clients including Residential, Adult Day Treatment and Case Management at a variety of locations.

  • Evaluates the need for development of alternate programs in response to client and community needs.

  • Maintains necessary documentation of services in compliance with agency policy and procedures, regulatory bodies, state law, contract specifications and professional standards.

  • Directs and coordinates the programs' CARF Accreditation compliance and survey process.

  • Prepares reports and documentation using agency software (CMHC, Excel, Word, etc.) as needed.

  • Completes electronic medical record documentation as required.

  • Writes, reviews, and modifies all policies and procedures related to essential documentation of services.

  • Represents Porter-Starke Services with community and state entities regarding the mental health needs of our clients.

  • Provides client advocacy, particularly with respect to personal safety and access to appropriate support systems.

  • Collaborates with internal and external health care providers to maximize and promote continuity of care.

  • Collaborates with Executive Vice President, Finance in the development and implementation of the annual budget for assigned departments.

  • Oversees the departmental budget, adjusting as necessary to maintain the fiscal health of the organization.

  • Ensures quality and integrity of the programs through performance indicators and quality improvement.

  • Acquires and maintains adequate levels of qualified staff to match the needs of programs and services.

  • Identifies, recruits, interviews and hires employees as directed and needed; plans, assigns, and directs work; appraises performance; rewards and disciplines employees; addresses complaints and resolves problems.

  • Provides overall leadership, supervision and training to all departmental staff.

  • Establishes, monitors and evaluates staff performance to support quality treatment outcomes.

  • Monitors and evaluates departmental systems and processes.

  • Ensures staff is educated on a continuing basis regarding compliance with documentation standards.

  • Promotes an environment that encourages staff growth and development by developing and supporting ongoing educational programs.

  • Ensures staff credential/licenses are updated as needed and shared with Human Resources.

  • Maintains records on in-services and other educational experiences in which staff participates or attends.

  • Supervises department coordinators.

  • Bachelor's Degree or equivalent in social work, psychology or related social services field is required.

  • Minimum of two years experience including direct supervision of staff, fiscal management, provision of clinical service (therapy) and program development required. Five to ten years' experience preferred.


MEDICAL OFFICE SPECIALIST – Location: Various

  • Schedules client appointments.

  • Makes appointment reminder calls to clients.

  • Pulls/prepares client charts for clinicians according to appointment schedule.

  • Performs check-in and check-out of clients.

  • Receives payments from clients for co-pays and payments on account.

  • Performs deposit of office cash receipts.

  • Contacts, schedules appointments and processes paperwork for referrals.

  • Initiates ROI forms for info requested by medical staff from other providers.

  • Ensures completion by medical staff of all appointment fee tickets/SALs.

  • Coordinates client requests for ROI and form completion.

  • Monitors no-shows/cancellations. Sends letters to clients as appropriate.

  • Requests/tracks needed health records.

  • Receives, sorts, forwards office mail.

  • Communicates inpatient consultation requests to medical staff.

  • Maintains updated schedules and medical staff on-call calendar.

  • Formulates reports for the department, associated committees and reports as requested.

  • Prepares forms and reports (i.e. terminations, schedule templates, release of information, etc.).

  • Provides clerical support (transcribing, copying, collating, form completion) for staff..

  • Maintains adequate supplies inventory (office, charting, etc.).

  • Medical office administration or healthcare experience and/or training preferred.

  • Adequate typing skills, speed and accuracy.

  • Adequate vision, hearing, speech and language.

  • Adequate mobility to sit, stand, walk, reach, kneel, stoop or bend.

  • Adequate digital dexterity to perform any function requiring use of hands and fingers.

  • Ability to lift and carry up to 20 pounds.

 

THERAPIST, CHEMICAL DEPENDENCY AND ADDICTIONS – Location: Valparaiso

  • Has knowledge of addiction and recovery in twelve step programs; human behavior and abnormal psychology; signs and symptoms of alcohol and other drug abuse and addiction; twelve core clinical functions, screening, intake, etc.; support groups in community and referral sources; behavior modification, treatment plans, relapse prevention and crisis intervention, case management and client education; general standards, policies and procedures of regulatory bodies, state laws, contractual agreements and professional standards.

  • Demonstrates skills in individual and group counseling for adults, adolescents and families; implementing and coordinating programs; reading and interpreting professional journals and technical procedures and regulations; presenting information and responding to questions from individual and groups of clients, customers, co-workers and the general public; typing and basic computer usage.

  • Provides chemical dependency intakes and evaluations or assessments.

  • Conducts group, family and individual therapy.

  • Performs case management for clients.

  • Monitors treatment progress of client.

  • Maintains open communication and good working relationships with referral sources.

  • Completes electronic medical record and other documentation as required by the agency's policies and procedures.

  • Participates in staff meetings to plan and/or review and update client's master treatment plan based upon their needs, progress and/or problems.

  • Provides education through presentations for local schools or community organizations.

  • Conducts individual chemical dependency assessments at Porter Hospital and emergency services at the Women's Recovery Home as a member of the Weekend Coverage Team.

  • Assists with general office duties when necessary (i.e. answering telephone, copying, etc.)

  • A bachelor's degree in social work, psychology or related field is required. A Master's Degree in Social Work is preferred.

  • One to three years' experience providing counseling services to alcohol or drug abusers in a clinical or community agency is preferred.

  • Current appropriate certification by Indiana Counselors Association on Alcohol and Drug Abuse (ICAADA) and National Association of Alcoholism and Drug Abuse Counselors (NAADAC) are preferred.

  • Recovering applicants should have a minimum of two years of sobriety.

  • This position requires employees to work evening and weekend hours.

 

THERAPIST, CHILDREN'S SERVICES – Location: Valparaiso

  • Provides individual, group and family therapy.

  • Attends student conferences as scheduled and meets with referral sources.

  • Completes all documentation and writes or dictates progress as required by the agency's policies and procedures.

  • Participates in staff meetings and/or peer consultations to plan and/or review and update a client's treatment plan based upon their needs, progress and problems.

  • Completes electronic medical record documentation as required.

  • Assists clients in meeting their treatment goals.

  • Provides education through presentations for local schools, community organizations and referral sources.

  • Connects clients to available community resources and acts as an advocate for clients.

  • Coordinates care with internal and external providers.

  • Attends team meetings and staffings as scheduled.

  • Masters Degree in counseling, psychology, social work or related human services field is required.

  • One to three years of experience in adolescent, pre-adolescent, family therapy or a related area is required.

 

REGISTERED NURSE (PART TIME) -- Location: Valparaiso Inpatient Unit

  • Delivers general and psychiatric nursing age appropriate care to adults 18 and above that are hospitalized for treatment of psychiatric or emotional disorders

  • Serves actively and constructively as a member of the multidisciplinary treatment, supporting the organization and program or unit philosophy of care:

  • Ensures the safety and well being of each patient;

  • Maintains a therapeutic environment.

  • Administers topical, oral, sub Q, IM medications; knows their action, purpose and side effects

  • Recognizes and takes appropriate action if adverse drug reactions occurs. 

  • Provides medication education that is age specific to patients with appropriate documentation.

  • Practices good interpersonal skills with physicians, patients, staff, guests and contact with others, i.e. referral sources. 

  • Facilitates age specific didactic groups such as coping skills, medication education, nursing group, communication skills, self-management, goals, and wrap-up.

  • Education--Graduation from an accredited school of nursing

  • Experience--1-2 years of experience in psychiatric nursing preferred (+) 1 year medical/surgical nursing preferred

  • Licensure--Current license for the State of Indiana, CPR certified

 

BEHAVIOR SPECIALIST (CHILDREN'S SERVICES) -- Location: Valparaiso

  • Provides direct client care by implementing programs and facilitating and monitoring groups.

  • Provides a safe, comfortable and therapeutic environment to ensure the safety and well being of consumers and staff.

  • Becomes familiar with each consumer's treatment plan relating to placement, interventions and goals and discharge plans.

  • Aids in planning, implementing and evaluating services to the consumers.

  • Prepares and maintains paperwork and other documentation (i.e. charting, daily report sheets, etc.)

  • Plans and/or participates in appropriate recreational activities to promote consumers' well being.  

  • Provides transportation for consumers to meet requirements or activities outlined in their overall treatment plans.

  • Maintains confidentiality of all consumers.  

  • Completes electronic medical record documentation as required.

  • Promotes a therapeutically healthy environment for all consumers.

  • Participates in team programs, staffings, meetings and special projects.

  • Develops and revises treatment plans as needed.

  • Enforces rules and regulations for appropriate conduct of consumers and visitors.

  • Transports clients when necessary.

  • Acts as a liaison to community and governmental agencies.

  • Complies with policies and procedures related to the care and well-being of all consumers, ensuring respect and dignity in treatment.

  • May be subject to threats and verbal abuse from consumers. There is potential for physical contact from consumers acting out in anger.

  • Requires Bachelor's degree in human services field.

  • Valid driver's license and ability to obtain public passenger chauffeur license.



PSYCHIATRIC TECHNICIANS (FULL AND PART TIME) -- Location: Valparaiso Inpatient Unit

  • Provides age appropriate direct patient care to adults 18 and above who are hospitalized for psychiatric or emotional disorders.

  • Ensures the safety and well being of these patients, supports and maintains the therapeutic milieu.

  • Monitors patient progress and communicates information regarding patient's age appropriate behavior and progress to other members of the multidisciplinary treatment team. 

  • Maintains communication with patient, family members and referral sources in patient's interest.

  • Facilitates, and co-facilitates didactic groups established and implemented to meet the age specific needs of the patient population that is being served. 

  • Encourages and facilitates patient's participation in therapeutic activities.

  • Supervises patients to ensure their safety and their involvement in treatment. 

  • Maintains a safe, orderly and therapeutic physical environment for patients of all ages.

  • Assists with completion of age specific psychosocial assessments and evaluations.

  • Communicates with patient post discharge to review compliance with follow up plans.

  • Provides input during shift report regarding patients progress to include behavior, mood, sleep, appetite, and ADL's

  • Carries out general nursing assistance appropriate to patient's age as assigned by charge nurse/manager, communicates and documents 1:1 interaction of day to day patient activities, communicates to other members of the multidisciplinary team.

  • Implements Program Secretary duties/order entry if designated and cross-trained.

  • Assists in the training and orientation of new psychiatric technicians

  • Education--Graduation from accredited high school or its equivalent, bachelor's degree preferred.

  • Experience--1-2 years acute care inpatient psychiatric experience

 

CASE MANAGER (ADULT) -- Location: Valparaiso and Knox

  • Provides case management services and in-home oversight.

  • Provides education in activities of daily living.

  • Works in a team setting to manage a case load consisting of individuals receiving any of the residential services, outreach services, or monitoring services necessary to help them live within the community.

  • Assists clients in personal financial administration.

  • Provides appropriate therapeutic interventions and crisis interventions.

  • Plans and organizes group therapy for those clients whose treatment plan calls for same.

  • Provides documentation of interventions per department standards.

  • Initiates, monitors and updates clients' treatment plans per department standards, and in cooperation with the treatment team.

  • Assists clients in meeting their treatment goals.

  • Provides appropriate discharge planning and transition to aftercare services.

  • Completes electronic medical record documentation as required.

  • Connects clients to available community resources and acts as an advocate for clients.

  • Coordinates care with internal and external providers.

  • Create group materials as needed

  • Attends staffings, clinical supervision, in-services and staff meetings.

  • Requires Bachelor's degree in human services field. 

  • Case management experience preferred.

  • Valid driver's license and ability to obtain public passenger chauffeur license.
