Employee Benefits
Eligible employees at Porter-Starke Services, Inc., including those at our Marram Health Center, are offered a competitive benefits package upon hire. A full-time regular employee is eligible for health, life, vision, and dental insurance. Coverage becomes effective on the first of the month following 30 days of employment. We also have competitive pay rates as well as 11 paid holidays and a generous 20% match of contributions to employees’ 401k account. - Medical plan options via UMR-Choice Plus Network, Traditional Plan or High Deductible Health Plan (HDHP)
- The HDHP can be used in conjunction with a Health Savings Account (HSA), with an employer contribution paid quarterly.
- Employees must work 60 hours per pay period to be eligible for medical insurance. Individual, Spouse, and Family rates available.
- Employees who participate in a medical plan are eligible to use free urgent care services in select locations.

IMPORTANT HEALTH COVERAGE DOCUMENTS
If you are currently or were previously employed by Porter-Starke Services, Inc. and enrolled in the Medical Benefits Plan for the 2025 tax year, you are entitled to a Form 1095-C, which serves as proof of the coverage we offered you. This statement was previously mailed to individuals; however, due to new regulations designed to reduce paperwork and administrative burden, we are no longer required to send paper copies via mail. You are not required to file this statement with your personal taxes. However, if you would like to request a free copy of your Form 1095-C for your records, you may do so by emailing hr@porterstarke.org, calling 219-531-3500 or sending a request to HR at 601 Wall Street, Valparaiso IN 46383. A paper copy will be mailed to the address on file within 30 days of receiving the request. If you prefer to receive an electronic copy of your Form 1095-C, please include your preferred email with the request. You may also access your Form 1095-C electronically by logging into your UltiPro account and navigating to Myself>Benefits>1095-C.
