Case Management

Our team of case managers, engagement specialists, and peer recovery specialists help people by linking them with community resources, assisting them with access to that care, providing information and resources, and coordinating care between multiple healthcare providers.

Case management is available to adults, teens, children, and families. It may be recommended based on an assessment by a Porter-Starke Services provider or through an outside referral.

Adult Case Management


Case managers work with people to create a treatment plan that provides the services, support, and skills training that encourage and develop the highest level of independence and functioning possible. Services are provided in-home or at a Porter-Starke Services location.

Skills training in activities of daily living can include budgeting, grocery shopping, and medication adherence. Additional training focuses on self-care skills, maintaining physical health, tobacco cessation, illness management, medication oversight, and crisis intervention. Support includes a Skills Training Group that develops wellness and cooking skills, experiential therapy groups, and housing support that links clients, when appropriate, with residential or community resources.

Child Case Management


Child case managers work with children and their families to help improve common issues like family communication, disruptive or aggressive behaviors, interpersonal conflict, and functioning at home and school. Working cooperatively with children, parents, guardians, teachers, and other healthcare providers, case managers ensure the services provided lead to the best possible outcomes. By helping coordinate services with families, case managers also help connect families with various community resources for additional care and assistance.
